Sample Report

To: Ms. James, Class Teacher
From: Aaron Joseph, Class Monitor
Date: 17th September, 2024
Subject: Report on Incident Between Two Classmates During Lunch

At approximately 12:15 p.m. today, Wednesday 17th September, an incident occurred near the school canteen involving two of our classmates, Mark Peters and Jason Ali. The situation took place during the lunch break while most students were seated in the lunch area.

According to what I observed, the disagreement began when Jason accidentally spilled juice on Mark’s lunch tray while reaching over the table. Mark became upset and raised his voice, accusing Jason of doing it on purpose. Jason immediately apologised, but Mark pushed his tray aside and stood up angrily. A verbal argument followed, attracting the attention of several students nearby.

Before the situation could escalate further, I intervened and called Mr. Thompson, the teacher on duty. He arrived promptly and separated the boys. Both students were taken to the Principal’s office for a discussion about the matter.

The outcome of the incident was that both students apologised to each other after speaking with the Principal and their parents were contacted. They have agreed to be more careful and respectful going forward. No further disciplinary action was required, and they returned to class quietly.

Signed,
Aaron Joseph
